Summary: During its descent at night and in poor weather conditions, the aircraft hit the Charleys Peak located 25 miles southeast from Alamosa. The aircraft was carrying one pilot, a flight paramedic and a flight nurse to pick up a patient in Alamosa. The pilot's failure to maintain clearance from mountainous terrain. Contributing to the accident was the pilot's inadequate preflight planning, improper in-flight planning and decision making, the dark night, and the controller's failure to issue a safety alert to the pilot.
